Use this online form to submit a referral to the DHS Division of Rehabilitation Services (DRS). DRS staff provide services to people with disabilities in 48 local offices located in communities throughout the state. Once you have filled out the form, your name will be sent to your local DHS-DRS office. A counselor should contact you to schedule an interview
